As you say ray he will be going over this in his mind for the rest of his life. I could not agree with the investigation more with regards to the fact that the interlocking should have been there with the built in safegaurds to stop this happening.

Going back to the Moreton-on-Lugg incident, Alan Williams absolutely shreds RAIB in the April issue of Modern Railways. The RAIB report took as one of its key premises the fact that they were unable to source an industry requirement that proper approach locking or other safeguard should be provided as the earliest document that they could find was dated 1988 and was unclear in its requirements; therefore there was no case to be made against Network Rail for failing to supply adequate safety systems. It took Alan Williams one call to the IRSE to source the 1963 version of the same document where it clearly lays out that approach locking must be provided for new works or alterations to existing schemes. The core policy was not re-written between then and 1988 so the 1988 document should implicitly accept that the approach locking is a mandatory requirement when works take place. Application of this requirement when the refurbishing works at Moreton-on-Lugg took place would have avoided this tragedy. He also notes that sources in Network Rail have indicated that boxes which have relevant track circuits (as is the case at Moreton-on-Lugg) could now have a software solution implemented for potentially as little as £5,000.

Looks like the net outcome of the Network Rail and the RAIB work is that the bobby is hung out to dry (behind a nice gloss of psychological factors, etc.) while there is no action to address the main issue of why mandatory standards were not applied.
