there is an incident somewhere back in this thread where a Meridian ran on for quite a way after the alarm was pulled (because the buffet car had derailed) as it was in a cutting, not sure if it was decided that the driver was correct in overriding it since it damaged a lot of track

Just for the sake of completeness, the RAIB published a report on a safety incident between Dock Junction and Kentish Town, which hasn't been included in this thread.

I know the report doesn't seek to allocate blame, but it made my blood boil on behalf of the passengers!

The RAIB is carrying out an investigation into a fatal accident involving a volunteer member of railway staff who was carrying out guard’s duties on the North Yorkshire Moors Railway. This is a standard gauge heritage railway running from Pickering to Grosmont.

The Rail Accident Investigation Branch (RAIB) has released its report into the fatal accident which occurred at Mexico footpath crossing, near Penzance, 3 October 2011. The RAIB has made five recommendations.

The Rail Accident Investigation Branch (RAIB) has released its report into an incident during which a train passed over Llanbadarn level crossing, near Aberystwyth, on 19 June 2011. The RAIB has made three recommendations to Network Rail, two to Arriva Trains Wales and one to RSSB.

The Rail Accident Investigation Branch (RAIB) has today released its report into an incident involving a runaway track maintenance trolley near Haslemere on 10 September 2011.

The RAIB has concluded its investigation into the accident at Liverpool James Street station on 22 October 2011.
The Crown Prosecution Service has asked that we delay publication of the report until after the trial of the train guard involved.

RAIB 2011 Annual Report
The Rail Accident Investigation Branch (RAIB) has released its annual report for the operational period 2011. It is published in two sections; Section 1 covers the work of the RAIB in 2011, and Section 2 covers recommendations that were the subject of a report by ORR during 2011. During 2011 the RAIB published 20 investigation reports, 6 bulletins and started a further 27 investigations.
