I've been waiting for this report to come out, it confirms most of what I'd heard.

The phrase "Rice Crispie drivers license" springs to mind :doh

A litany of failure. Frankly I wood-n't trust them with one of these:

(Though on a separate note, I may have to write to ASDA asking why there's an LC over a siding. You've got to get best practice taught early!)

Thanks to all who post details of the RAIB reports. However, it would be nice if we could have a consistent format rather than some links referring to the RAIB summary page and others directly picking up the report. For myself, going via the summary page just adds an extra click into the process when I am trying to read what happened.

I've never bothered while there is such an excellent service here.

I believe the "v2" might be due to a fairly significant error, in "v1".

The RAIB has released a revised report of the above:-
"The Rail Accident Investigation Branch (RAIB) has released an update to its report into an accident involving a pantograph and the overhead line near Littleport, Cambridgeshire on 5 January 2012. This update includes a new addendum covering difficulties that passengers had in operating the Passenger Emergency Alarm (PEA) handles that were reported to the RAIB by a passenger after the original report had been published. Actions taken by the train operating company to address these difficulties mean that no additional recommendations have been made."
