This might be a case of "working as designed"...

When a TC failure occurs before the first controlled signal at an entry point (eg between N183 and N187 from Severn Tunnel Junction) the signaller doesn't get a call from the driver to say the train is stood at a red signal.

I'm guessing in real life that call would go through to the Newport signaller. So that's correct in the sim? But on the sim a call from N187 would come through to the (Bristol) "sim signaller", so not consistent.

Anyway, in the above described condition, there's no genuine way to get a train past the TC failure (without direct intervention through the timetable.)

In real life, I'm guessing the signallers would collaborate to resolve the situation?

I do not know whether the issue was ever resolved or not

If it's a Newport signal the train is stopped at then the call will go to Newport not Bristol.

I'm a little confused by what you say here about inconsistency,
"I'm guessing in real life that call would go through to the Newport signaller. So that's correct in the sim? But on the sim a call from N187 would come through to the (Bristol) "sim signaller", so not consistent."
Can you expand on that please.

Can you expand on that please.

You don't get calls from N183
You do get calls from N187

I assumed that as both these signals have N plates, then calls from both should go to Newport signalman in real life. This may be a bad assumption on my part.

I think the reason is that N187 is controlled, and has effectively been 'adopted' by Bristol PSB for the purposes of the simulation, whereas N183 is an external signal.

Correct! In general we try to avoid failures outside the area (or just inside and not under our control) so we don't get this kind of situation. Omissions happen.
