I was considering the way that SimSig handles ACOAs. At the moment it seems to assume that any ACOA is an error and consequently gives a penalty by way of a time delay to the train (and a score hit as well). I realise the way that ACOAs are dealt with by the sim is a bit of a hack (no offence intended to Clive et al) but perhaps it would be more prototypical to give the user a list of reasons to give to the Driver if they need to put back for a genuine reason (rather than making a mistake). Something like:

* Sorry Driver, my mistake. Please continue after speaking with your Control.
* Sorry Driver, I had to cancel the route because of a problem ahead. Please wait five minutes for the Signal to clear.
* Sorry Driver, I had to cancel the route because of a problem ahead. Please examine the line and report at the next signal.
* Sorry Driver, I had to cancel the route because of a problem ahead. Please pass the signal at Danger, examine the line and report at the next signal.
* Sorry Driver, I had to cancel the route because of a problem ahead. Please wait 15 minutes for the signal to clear.

I think those five exlanations to the Driver cover most eventualities. The first one would give a penalty, as now, but the others, being genuine reasons for a signal to be replaced, should not (except perhaps a short delay to allow for the Driver filling in a form if he needs to, but nowhere near as long as the delay for a "mistake"). Of course, I realise that people could "cheat" by using one of the no penalty options rather than admitting a mistake, but maybe we can trust people not to do that!

Does anyone know if a driver stopping immediately to report ACOA is a UK rule book requirement, or is it a SimSig-specific penalty? What are they reporting?

If there was no SPAD and everyone is okay, why not continue and report at a more appropriate location?

I'm not au fait with the current Rule Book, but I would think that the Driver would stop, even if only to query the signal change. All of the reporting and so on can probably be done later. There will almost certainly be a Driver along in a minute to explain.

They must stop and report to the Signaller but there is no requirement for them to contact or seek permission to proceeed from anyone else. Notifying the TOC control is the responsibility of NR control, who in turn are notified by the Signaller.
