If a TC remains occupied after the passage of the last train and it’s holding points in reverse, is the fact it’s holding points confirmation that it’s not just an indication failure?

If the control system has not received updates from the interlocking then there should be an alarm. How you deal with that, or with a track section that appears to be a failure, is beyond the scope of a simulation forum given that you posted this in "The real thing (signalling)" and you appear to be a signaller-in-training.
