21: If you mean identical names then I'd say Charing Cross (London and Glasgow) but otherwise possibly Barry (Glamorgan and Angus).

Friedrichstrasse U-Bahn station in East Berlin prior to the fall of the Berlin Wall. It was on a transit line between the northern and southern regions of West Berlin and had a Mitropa duty-free shop for Westerners with hard currency.

Peter, I'm after identical place names, not "identical names". I thought you had beaten me with Barry (Barry Island - Barry Links), but that is only 353 miles! Keep trying.

12: c? But I don't fully understand the question
c. Bowling – Bat & Ball and Alexandra Palace – Alexandra Parade

Bowling and Alexandra Parade are in the Glasgow Area, whereas Bat&Ball is South of London (possibly Kent) and Alexandra Palace in North London.

Not sure I know where all of the others listed actually are.

Well done, 01 002 (Holyhead Break water) was withdrawn in 1981 and never got a corporate paint job. I thought that one would take a little longer to solve.

Peter,

I thought you had beaten me, but the photos I can find of D200 in the 1980's show the lion on a crown holding a wheel emblem, not unicycling.

I Could give you that one, but even the city council seems to have dropped the "Kingston-upon" part (or the station ignored the town and is named after the river).
