Seems like the report was not proof checked before publication....if it was London Midland have a serious overcrowding problem and must now hold the record for the number of people crammed into a service train......

2000 passengers on a 12 coach 350. Average of 166.66 passengers per coach. It's not exactly an unrealistic number.

Quite so - the seating capacity for such a train is supposedly 800, and booked into Euston at 8.33am on a weekday, so I doubt there was much free room at all (perhaps rounded upwards to 2000, though).

According to Wikipedia, seated capacity is 229 per 4-car train, so 687 for 12 cars. So 2000 passengers would mean 109 standees per car which seems implausible even if it was crush loaded.

The Wikipedia numbers are supposedly 350/1, with 350/2s having a denser seating layout (which is acknowledged further down on the same page!)

However, it seems that overcrowding factors beyond 150% are extremely rare, and that >200% overcrowding only occurs in very few cases: https://www.gov.uk/government/uploads/system/uploads/attachment_data/file/541345/top-10-overcrowded-train-services-england-and-wales-spring-and-autumn-2015.pdf

They've not done a full report, just a safety digest. Safety digests are often used when the only safety learning is compliance with existing rules and procedures. These are done as HTML pages as you've linked above, there is no PDF to find.

I did a very small piece of work with RAIB last summer where I suggested including some comments from the inspector that wrote the report. Pleased to note they've started doing that, albeit by the Chief Inspector (which is probably better!), on the report's HTML page.

Anyway, in his remarks, he made some very pertinent comments relating to Glanrhyd - having read the Lamington report seems it had all the hallmarks of a repeat but for the switched on driver who reported the difference in track profile - not sure I'd be as alert at work before 7am!
